9. Fotocopy tanda Lunas PBB Tahun berjalan
10. Surat Kematian Asli ( Bagi duda / janda cerai mati )
11. Surat Cerai Asli ( Bagi duda / janda cerai hidup )
12. Foto Berwarna Baground Biru
